Preparing for your first session
If you are a new client, I will send you a link to my patient portal. You will need to fill out forms on the portal prior to coming to your first session.
Completing these forms in advance of our first session will help us make the most of our time...Thank you in advance for your cooperation.
If you would like me to coordinate my treatment of you with another provider (for example, your psychiatrist, primary care physician, etc.), please complete this form and return it to me, which will authorize me to release your psychotherapy information to your provider:

Your first session
I can meet with you virtually through a secure, HIPAA-compliant portal or in person, whichever you prefer.
If you wa,nt to meet in person, my office is located at 2998 Eskridge Road Fairfax, Virginia. The office is conveniently located near several infertility clinics. If you use public transportation, my office is a half mile south of the Dunn-Loring Metro Station (Orange Line). If you drive, there is free street parking adjacent to my office.
